  • Program information may be made available in languages other than English

Filing a Discrimination Complaint

How to File

To file a program discrimination complaint, you have two options:

Complete USDA Form

Complete the USDA Program Discrimination Complaint Form, AD-3027, available online at "How to File a Program Discrimination Complaint" or at any USDA office.

Write a Letter

Alternatively, write a letter to USDA that includes all information requested in the complaint form.
